Coverage Options

Short-Term Disability (STD): Weekly maximum benefit of $1,500, multiple benefit percentage options, and pre-existing condition benefit.
Long-Term Disability (LTD): Monthly maximum benefits up to $7,500 ($10,000 for select industries), 60% benefit percentage, 60, 90, and 180-day elimination periods, “Own Occupation” definition of disability – 36 months (60 months for select industries), pre-existing condition benefit, and rehabilitation and survivor benefits.
Life/AD&D: Guaranteed issue amounts from $50,000 to $100,000 depending on group size, maximum benefits of $200,000. All plans offer an equal amount of Life/AD&D.
Dental/Eye Care: Three comprehensive dental plans with PPO options, adult/child orthodontia coverage, and eye care option.
Critical Illness: Three plan design options with coverage up to $20,000 maximum. Three illness categories, subsequent occurence benefit, recurrence benefit, and lifetime maximum of 200% to optimize health and supplemental coverages.
Accident: Guaranteed issue coverage, three plan design options, 24-hour or non-occupational coverage available, and AD&D coverage can be included. Helps with high-deductible healh plan coverage.

Broker Bonus Program

The 2025 SmartChoice Premier Producer Program offers additional earning potential:

New Business Tiers: Earn 2.5% to 4.0% of new earned premium based on coverage volume.
Renewal Business Tiers: Earn 1.5% to 3.0% of earned premium based on case persistency.
