• Bronze PPO 5500/65 – available on the Full PPO network and Tandem PPO network.
• Bronze PPO 6500/70 – available on the Full PPO network and Tandem PPO network.
• Silver HMO 2750/65 – available on the Access+ HMO network, Local Access+ HMO network and Trio HMO network.
• Bronze Trio HMO 7000/70 – available on the Trio HMO network.

Jet Dental® Pop-Up Dental Clinics

Effective immediately, Humana is partnering with Jet Dental to offer onsite pop-up dental clinics to employer groups. At no cost to the employer, members have easy and convenient access to onsite dental services. This service is available to all commercial groups with Humana Dental PPO plans.
